When the total reaches 10 or more, or item 9 is nonzero, the scored result is flagged for review. Writeback is tested in the athena Preview sandbox with synthetic data. Routing to a clinician inbox is on the roadmap.

What the alert means

Quiet for everyone else. Flagged for the few who need eyes.

High total

Fixed PHQ 9 rules flag a total of 10 or more, and raise priority at 15 or more.

Item 9 nonzero

Self harm ideation on the form raises priority. Pilot practices must attest to an on call escalation path outside athena for these cases.

Does not diagnose. Does not replace clinical judgment. The patient form is designed to show 988 guidance when item 9 is nonzero. The app does not monitor patients in real time.
